A COUNCIL health and safety manager was unaware a safety committee had been set up until the fatal accident involving an inflatable artwork, an inquest has heard.

Denver Meade was appointed by Chester-le-Street District Council several months before the Dreamspace tragedy claimed the lives of Claire Furmedge and Elizabeth Anne Collings, and injured several other people, in July 2006 when it broke from its moorings at the town's Riverside Park.
